You can apply for your CapTel phone through the State of Oregon Telecommunication Devices Access Program (TDAP).

Please be advised that people who do not have a severe to profound hearing loss will not qualify for the Oregon CapTel’s long-term loan program.  If you have a mild to moderate hearing loss, please contact puc.tdap@state.or.us or 1 800 848-4442 and ask for the procedures to purchase a CapTel phone.
